Re: good light
I don't know anything about heat lamps since my short time with them led to me getting burned, melting something and zapping humidity but I am sure someone else does. For heat most of us use and recommend the ultratherm uth placed on the back of the tank. The brand is priced fantastically even with shipping since not available in store and is insulatable if necessary.
Here is a link to them if you are interested. You would buy the size that covers as much of the back of your tank above the substrate strategy level as possible. I have a 125gal tank so I have 2 on mine to cover the entire back.

Re: good light
Any of them can break this way! The glass is fused to the metal so it's a weak point - a good bump, age, or even just it getting hot can be enough to cause it to happen.
I personally like Fluker's brand bulbs the best. The glass is thicker and is colored (as opposed to just coated), so I've found them be more sturdy.
